This is my setup:

6/3 from a 50a breaker in a intermatic pushmatic box through a 3/4" pvc stub and LB into a Connecticut Electric Disconnect. Ground is to the ground bar on the panel, neutral is on the neutral bar on the panel, and each hot is on each side of the breaker.

The ground is grounded to the ground bar with the GFCI ground. The neutral is on the neutral bar with the big relay thing in the upper left corner. The hots go into the bottom of the disconnect.

I have 4 wires going through 3/4" PVC buried 18" approximately 15 feet to the spa. The PVC comes up through the bottom of the spa, into a 3/4" LB, and then 3/4" PVC into the control box. The ground is tied into the ground bar, the neutral is tied to the neutral wire that used to go to the GFI, the hots go directly to the bottom of the heater relay with one tied into the hot leg of the GFI.

The GFI on the disconnect reads 120v on the line portion. If I push the test button, it then reads 120v on the load portion, which leads to that top left relay. The problem is, that relay in the disconnect never clicks on and does not provide power to the spa. The only way to power the spa is to push the button in the middle of that relay, connecting the power to the spa. When this is pushed and held down manually, the spa works great.

Did I wire something incorrectly?

Sounds to me like the contactor has a defective coil.

You may want to disconnect power and test for continuity thru the contactor coil connections.

What should I be looking for across which terminals? Thanks!

I would say less than 1000 ohms. If you measure a lot greater than that, (or get an infinity reading - eg nothing), then the coil is open.
